This attestation is completely unnecessary. It is unclear what DWC's intention was. The purpose here is limit reimbursement to QME. What if the number of page is far below the actual number? What if declaration isn't provided, now physician office needs to call the attorney office and request it, another delay. How much more staff would physician's office need? Another useless red tape.

We need it so there is no payment dispute. It should be very simple to get paid. No complexity no fraud etc
