§ 59A-30.009 Recertification of Expert Medical Advisors.

(1) The Agency shall notify the expert medical advisor of the end of his/her two year certification period at least 90 days before expiration of that period and shall provide an application form AHCA Form 3160-0021 for recertification of expert medical advisor to the physician.

(2) The expert medical advisor shall be certified for a two year period. To be recertified as an expert medical advisor, a physician shall submit application form AHCA Form 3160-0021 to the Agency. Application form AHCA Form 3160-0021 is incorporated into this rule chapter by reference in subsection 59A-30.003(1), F.A.C.

(3) An expert medical advisor applying for recertification shall submit a completed, typed, signed application to the Agency at least 30 days before the end of his or her expert medical advisor certification period.

(4) An expert medical advisor applying for recertification shall provide proof of attendance at an Agency sponsored or Agency approved workers' compensation training workshop within the two years before the end of his or her expert medical advisor certification period.
