Jon you describe CPT 95852 as grip strength testing but the code seems to be defined as ROM of the hand. Aren't these two different things
Also when billing 95851 for each extremity is it 2 units for the left and right upper extremities or just 1

Reading the Coventry contract it is silent on 3rd party arbitration. It only speaks to dispute resolution and arbitration against Coventry itself not against a payer therefore IBR would control. A specific date of service is one week of FRP billed with one CPT code with 5 units.
